Under PHIA you have the right to make a complaint if you think your personal health information has been used, viewed, or disclosed inappropriately. You can read our Privacy Complaints policy and review the form that needs to be completed to initiate a privacy complaint.
Copies of Health Records may be released to patients, health providers, or other organizations upon completion and approved signatures on request forms available on this site.
